Application and Instructions

1. Click on the link below to open the application. You will need to unzip this file. This file requires Windows.

      - Application

2. Choose "Run." You will see a security window. Choose "Run" again. Click "OK"    
    on the OmniForm Page

3. Fill out the application.

     - NOTE: There are four (4) pages. The tabs are found at the bottom of the 
        OmniForm window.

4. Save it to a folder on your computer.

5. When you save, it will create an .exe file and an .html file.

6. Please attach the .html file to an email, and send it to bookstore@MissouriState.edu

    - NOTE: The University email system will block .exe files, so you must attach the .html file only.
